Learning through action to reduce infection is an action tool for clinicians and risk managers to use when a patient has a confirmed life-threatening infection, such as MRSA bacteraemia, Clostridium difficile (C.Diff)).
Based on the Root Cause Analysis toolkit it helps to clinical teams to identify what factors or events led to the infection, and how to reduce the risks of it happening again.
The tool uses a simple three-step process:
- React – what to do when notified about an infection
- Record – what were the main contributory factors/root causes?
- Respond – what needs to be done to prevent it happening again?
The results will help your organisation to gain a better overall understanding of the source and contributory factors associated with severe infections and take action to reduce the risks of them occurring elsewhere in the future.
