Community nursing and therapy service

Community nursing and therapy services provide care to people in the person’s own home. This includes the management of chronic health needs (such as diabetes and heart disease) as well as palliative care, health promotion and the provision of home nursing equipment to support self-care.

 

Therapy services may include physiotherapy, and speech and language therapy given in the community.

  

Community services are usually run as a provider service of a primary care trust. These services help reduce hospital outpatient appointments.

 

Patient safety incidents relating to community nursing, medial and therapy services reported to the Reporting and Learning System (RLS) include poor communication between practitioners and lack of access to equipment.
