This learning from reporting section, taken from Quarterly Data Summary Issue 7, focuses on medication incidents in primary care.
There were 4,071 incidents reported to the National Reporting and Learning Service by primary care organisations between October 2006 and September 2007 related to medication errors. The four most common themes were vaccination, prescribing (including administration errors at production of prescription), repeat prescribing and dispensing.
The document provides examples of typical errors reported for each theme and considers some of the problems, issues and risks associated with each.
