Review of Intrapartum–Related Perinatal Deaths Pro Forma

This comprises a pro forma for any professionals involved in the review of intrapartum-related perinatal deaths and may also be used for the review of all perinatal deaths. The aim is to find out any avoidable factors, identify any lessons to be learned and then to develop and disseminate an action plan. The pro forma should assist in, and be used in association with,  Root Cause Analysis.
