The development of this checklist has been undertaken as a collaborative project between the NPSA and the Royal College of Obstetricians and Gynaecologists.
This checklist is adapted from the WHO surgical safety checklist to ensure it is relevant and applicable to maternity care and that all key clinical risks in this environment are addressed. This adaptation uses the same principles but with maternity specific prompts, acknowledging that the woman (and her birth partner, if relevant) are involved in the process.
Local adaptation of this checklist is encouraged to ensure it is effectively integrated into clinical practice (and a Word version is provided for this purpose). There may be additions, and items can be moved (for example to the ‘team brief’) and integrated with local documentation. Any adaptations should be undertaken in accordance with your local governance scrutiny process.
