WHO Surgical Safety Checklist: for radiological interventions only

This guidance should be used in conjunction with the WHO Surgical Safety Checklist Supporting Information document.


The development of this Checklist has been undertaken as a collaborative project between the NPSA and Royal College of Radiologists. This Checklist is adapted from the NPSA WHO Surgical Safety Checklist to ensure it is relevant and applicable to radiology and all key clinical risks in this environment are addressed. It should be used for all patients having a radiological intervention.

 

The Checklist should be introduced into radiology departments as part of a wider process which includes a ‘team brief’ before the list starts and a ‘team de-brief’ at the end of the list.

 

Local adaptation of this Checklist is also encouraged to ensure it is effectively integrated into clinical practice.  This may mean that some of the interventions are moved to the ‘team brief’.  Any adaptations should be undertaken in accordance with your local governance scrutiny process.

 

As part of the ‘Sign In’ there is a requirement for the procedural site to be marked. For interventional radiology procedures that involve laterality the site marked should be the symptomatic side. 


Also part of the ‘Sign In’ is the requirement to check the patient’s risk factors for renal failure. The rationale for this is that the administration of contrast media can have serious complications for some patients, i.e. for those with pre-existing renal insufficiency, diabetes mellitus and those aged over 75 years.
