This learning from reporting section, taken from Quarterly Data Summary Issue 13, focuses on the problems surrounding the insertion of chest drains.

The National Reporting and Learning Service (NRLS) published a Rapid Response Report (RRR) on the risks surrounding the insertion of chest drains in May 2008, but a review of this RRR in June 2009 showed that whilst it had raised awareness amongst clinicians, just over a third of eligible trusts had still not completed the actions outlined.

This feature reinforces the serious nature of the problem, outlines what NHS trusts and health professionals should be doing to address it, provides an example of good practice, and shows how the NRLS are building on the initial review.
