The NPSA releases Organisation Patient Safety Incident Reports data for individual NHS organisations in England and Wales.
The data for the March 2012 release covers patient safety incidents that occurred between 1 April 2011 and 30 September and were reported to the National Reporting and Learning System by 30 November 2011.
