Notes from previous data releases

The allocation of Primary Care Organisations to either  the ‘inpatient ‘or ‘no inpatient’ cluster is dependent upon which denominator data is available irrespective of whether the organisation has inpatient provision or not.

This organisation has been classed as a low reporter since the number of incidents that occurred between October 2008 and March 2009 was 10 or less. We have not produced a full report for this organisation as detailed feedback on a small number of incidents would be misleading. For example, an organisation reporting four incidents that resulted in low harm and one that resulted in severe harm would appear to have 20 per cent of their incidents classified as severe.
