HEART OF ENGLAND NHS FOUNDATION TRUST

This organisation has been classed as a low reporter since they
missed the deadline for submission of data for the period of
October 2008 to March 2009 to the Reporting and Learning
System (RLS). This was due to technical difficulties in submitting
the data. As such a full report is not provided for this organisation.
However, as the organisation is committed to supporting the RLS,
these incidents have been reported retrospectively. With the
help of NPSA, the organisation has now produced a summary
report which can be used in conjunction with the NPSA report
to provide a comprehensive report. The organisation's summary
