The National Reporting and Learning System will continue under the auspices of the NHS Commissioning Board. We would like to reinforce the importance of NHS organisations continuing to report patient safety incidents in the usual way through the National Reporting and Learning System. This is so that trends in safety incidents can be identified and acted upon as early as possible.

Patients Reporting an Incident
If you are a patient who wants to file a complaint or report an incident, unfortunately, the NPSA is unlikely to be able to assist you as it is not within our remit to give advice on individual cases.
The Department of Health's NHS complaints guidance, in the first instance, recommends that you contact either the Patient Advice and Liaison Service (PALS) or the complaints manager at the NHS trust, who may be able to resolve your concerns on the spot, or alternatively can provide you with details of how to complain or raise concerns.
